CONTINUUM OF CAREAdoption Services Non-Residential ServicesOutpatient: Visits are one hour or less. The number of visits per week depends on the youngster's needs. In addition to assessments, several forms of treatment are available: individual psychotherapy, family therapy, group therapy, and medication monitoring. Partial Hospitalization: This intensive treatment program provides therapeutic intervention as well as individualized education supports five days per week. An after-school component is also available for adolescents. Intensive Case Management - Resource Coordination: Specifically trained individuals coordinate psychiatric, financial, legal, and medial services to help the child or adolescent live successfully at home and in the community. Family Functional Therapy Program: Specialized short-term family therapy program for youngsters' aged 11 - 18 at risk or presenting a broad range of acting out behaviors and referred by the juvenile probation/court system. Recognized as a "model program" being replicated in Luzerne/Wyoming counties, it is one of the "blueprints for violence prevention" programs in the nation. Home Based Intensive Family Services: A team of specially trained staff go into the home and develop a treatment program to help child or family. Emphasis is on preventing out of home placements. Mental Health Evaluations: Per continuum description. Medication Clinic: Child and adolescent psychiatrist does medication assessments; informed consent by care givers with regular doctors visits and adjustments to ensure proper medication and dosage. Trained psychiatric nurses who monitor symptom response and seek family and school feedback regarding response to medication and assist doctors. Coordination and consultation with the child's primary care phsyician is emphasized. Psychological Testing: Provided when diagnostic clarification is needed. Testing is provided under the supervision of clinical findings and a licensed clinical psychologist. Recommendations benefit the treatment team's planning to enhance therapeutic outcomes for the child and family. HMO Contracted Mental Health Services: Mental health specialists provide mental health assessments and interventions as approved by HMOs both locally and nationally. Services include outpatient services, psychiatric evaluations, psychological testing, medication assessment and monitoring, partial hospitalization services, and crisis response teams depending on each contract. We recently entered into shared risk contract with First Priority Health. Critical Incident Response Team Contracts: Staff have expertise in trauma response and, upon request, will contract to do prevention training, postvention work in schools, the community at large, and at work sites. Parent Educational Services: ADHD Parent Education, Oppositional Defiant Children's' Parent Group, 1-2-3 Magic Parenting Group, Active Parenting Group. Additional and diagnoses specific psycho education is provided to caregivers throughout our continuum of care. Residential ServicesTherapeutic Foster Care Program: A surrogate family (professional parents) provides a therapeutic environment and works with CSC therapist's in treating emotionally disturbed children and adolescents in a family home environment. CRR Group Home: This home provides a safe and supportive home environment for seriously disturbed adolescents. Twenty-four (24) hour staff supervision, mental health support, regular system of mental health re-evaluations, and therapy are provided. Therapeutic Respite Services: A client stays briefly away from home with specially trained individuals in a therapeutic environment that promotes resolution, stabilization and return home. Base Service UnitIntake, Assessment and Referral: Mental health professionals who register families help with determining the cost for services, and gather important family history and concenrs which are shared with our assessment team. This team consists of psychiatrists, psychologists, and other mental health professionals who recommend an individualized course of treatment to best suit the family. Intake staff help with ensuring the families are connected with these recommended services. Administrative Case Management: Base Service Unit staff monitor clients throughout their involvement in the continuum of care, as well as when client's needs require services outside this continuum, such as in other community based treatment resources, inpatient care, or residential treatment facilities. Community Based Services24-hour Crisis Services: Services include mobile teams who respond to crises in the community, provide in-person assessments, secure respite services, in-home diversion services, and inpatient referrals as necessary. In Home Diversion Services: Specially trained individuals stay in the home with the youngster and care giver to calm a crisis and prevent unnecessary hospitalization. Community Crisis Response Teams: Mental health professionals who respond to community disasters such as a school suicide, school violence, accidental death of a peer or teacher, and natural disasters at the request of a school or community group. Autism Program: Provides intensive therapeutic interventions to youngsters diagnosed with Pervasive Developmental Disorder. Provides assessments, intervention programs in home and/or school, parent support, a socialization service after school hours, and expert support for schools and families in educational planning for each individual child's needs. Behavioral Health Rehabilitation ServicesBehavior Specialist Consultation: Individualized behavior programming by a licensed clinician that designs models and interventions to change inappropriate behaviors. Mobile Therapy: Mental health professionals provide individual psychotherapy services to children and adolescents in the home and community. Therapeutic Staff Support Services (TSS): Operating as necessary in conjunction with either Mobile Therapy or Behavior Specialist Services, TSS professionals work one-to-one with children and adolescents to provide immediate intervention to change targeted behaviors. Specialty Community Services: Available by contract. International and Domestic Adoptive ServicesCSC offers comprehensive adoption services to individuals interested in adopting children internationally and domestically. CSC's adoption team will walk hand-in-hand with the family through the adoption process and continue this support for as long as the family needs it. We are a family focused agency who recognizes the unique and complex issues related to adoption and cross culture families.
